2. A copy of the latest nonprofit corporation annual report.


3. A draft copy of the issuer’s security.


4. A copy of the executed underwriting agreement.


5. A copy of the executed trust indenture.


6. Copies of all advertising to be used in connection with the offering.


7. A copy of the prospectus.


8. An originally executed and originally notarized Consent to Service of Process, FORM U-2, appointing Alabama’s Secretary of State to accept service of process.


9. An originally signed and sealed Corporate Resolution, FORM U-2A.


10. Provide list detailing Alabama registered restricted agents or broker dealers.


11. Complete NASAA Statement of Policy Guidelines Regarding Church Bonds and Church Bonds Cross Reference
